Consumer Access to Health Records & Complaints (Texas)

Under Texas law, you may request your health care records, contact the licensing authority for this practice, and file a consumer complaint.

1) Request your health care records from this practice

To request your records, Email your request to Info@getrootedrising.com and include:

• Your full name (and former name, if applicable)

• Date of birth

• A description of the records requested

• Where you would like the records sent (secure email or mailing address)

• A phone number in case we need clarification

Response timeframe: Requests are typically processed within 10 Business Days.
Fees : Electronic copies are typically provided at no cost. Fees may apply for paper copies or unusually large or complex record requests, consistent with Texas law. Any applicable fees will be discussed in advance.
